Attention:
This email is for anyone responsible for gathering new employee information for international students and non-US citizen employees.
Failure to follow these instructions will result in delayed payment and/or incorrect tax withholding for your employees.
International students and non-US citizen employees who have already completed the UNT Foreign National Information Form DO NOT need to fill out a new one unless their immigration status changed or has been extended.
Attachments:
The Foreign National Information Form must have the following attachments based on immigration status:
• F-1: copy of US Visa, I-94, and I-20
• J-1: copy of US Visa, I-94, and DS-2019
• H-1: copy of US Visa, I-94, and I-797 (Notice of Action)
• LPR: copy of Permanent Resident Card (I-551)
Or copy of temporary Permanent Resident stamp in passport.
Other immigration status may require additional or different documentation.
Routing:
Please send the following new hire packet documents to the correct departments:
Payroll:
• Foreign National Information Form (with appropriate attachments listed above)
• W-4
• Electronic Funds Transfer of Salary (if chosen)
Human Resources:
• HIPAA (if applicable)
• HRM-1A (EBD form)
• I-9 (with attachments)
**If the forms go to the wrong department it will delay your employee’s payment.**

Treaties:
In addition, many international students, teachers, and researchers may be eligible for tax benefits based on a treaty between their home country and the US. The most recent list of available income tax treaties is available at: http//www.irs.gov/businesses/international/article/0,,id=96739,00.html
Not all treaties listed contain student and/or teacher/researcher benefits. If an individual is eligible for a tax benefit under a tax treaty, they need to complete an IRS Form 8233. Come to the payroll office to obtain the form and its attachments.
